    Hi,

    could you please tell me how you call the form which must be completed by an employee if he/she wants to visit a doctor during working hours and which is submitted to the employer?

    But if I say "leave application form", how do I recognize that it is for the purpose of visiting a doctor?

    Maybe a Medical Leave Application Form ? I am not sure how many different kinds of leave application forms there could be in the work place.
    Where I work we submit our applications online.
