    You should say one of the following:
    What does 'not at all' mean? (I prefer this one.)
    What does not at all mean?
    What does not at all mean?

    You have to highlight the words that you are asking about, and you should start your question with the question word, in this case "What".

    'not at all' means not even a little, not any.
    Examples:
    A...Are you cold?
    B...Not at all.

    A...I hope I haven't caused you any trouble.
    B... Not at all.
