Cool self-infllicted twist of the knife

Could you tell me de meaning of "self-infllicted twist of the knife"; it seems an idiom, but...

The sentence where it found: "By the time their ordeal ends, the relief is punctuated by a self-infllicted twist of the knife."

    Thank you.

Cool Re: self-infllicted twist of the knife

Oh! I think I've just caught the meaning myself. It is something like making torment on oneself, like twisting the knife on one's wound. Isn't it?
