    How does he look?

    Hello,

    Is the question "How does he look?" asked only about a person's condition after an illness? Can I say: "How does your sister look after the holidays? - She looks fine. She always has a good holiday in the country."

    Yes you can. There's no risk of 'look' being interpreted as transitive - as in the old joke:

    A: My dog has no nose.

    B: How does he smell?

    A: Terrible.

    'What does he look like?' sounds slightly more natural to me, for the context you give. But 'How does he look?' is fine. You could also imply the question, by saying 'I bet he looks great...'

    The context though is suspect because the implication is that the vacation did her health, physical or mental, some good. Not that she was ill, but more so that the time away made her feel better than she felt before going on vacation.

    A: How do I look? <trying on a suit>
    B: Fabulous. You'll get the job if you wear that suit.

    A: How do I look? <returning after several years away>
    B: Tired. The stress of prison life will do that, though.

    A: How do I look? <after three days in Hawaii>
    B: Very tanned. It makes you look healthy.

    A: How do I look? <trying on a Cat in the Hat hat>
    B: Like an idiot. Take it off.
