    Your first sentence may be correct.

    But, I am sure, your second sentene is incorrect. Because we use the structure "have + been + past participle" only in passive sentences. Your sentence is active voice so the second sentence is incorrect.

    Your both sentences are correct except the place of adverb "yet" may be wrong.

    The first sentence is a Passive voice and the second one is a Active voice.

    I would have writte the sentences as :
    Please note that the contract has not yet been made for the following personnel by us. [Note: here the "by us" is not necessary, it can work well as you have written]

    Please note that we haven't yet made the contract for the following personnel.

    [Note : I am just a beginner]
