    passive or active

    I always get confused when I am going to make a passive or active sentence . for example I was going to ask my surgery wheather my blood test have been sent by hospital or not and I asked
    has my blood test result sent to you?
    or should I have said
    has my blood test result been sent to you?
    or maybe neither .

    The correct phrase is:
    Has my blood test result been sent to you?

    Do you need an explaination on the grammar of the active and passive voices?

    In the active voice, the subject is performing the action.
    This can be seen in the example I kick the ball
    I is the subject- and it performs the action of kicking - the sentence is active.

    In the passive voice, something else is performing the action.
    As in the case The ball is kicked by me.
    The ball is the subject - and it does not perform an action - the sentence is passive.
