    Which sentence is correct?

Phenobarbital level is therapeutic, drawn in May of 2007.

    Is this okay?

    or should it be

Phenobarbital level was therapeutic, drawn in May of 2007.

I am a medical transcriptionist, and this is the way the doctor dictates this.

Well since it took place in the past..."was" would seem to be appropriate.
