Smile break out in a cold sweat

When will you break out in a cold sweat?
I'll break out in a cold sweat if I've to speak in public and don't know what to talk about.

    Hi,

Do both of the above sound right? What does "break out in a cold sweat" mean exactly? Thanks.

--I feel, When do you break out in a cold sweat? sounds better.

--Second option sounds right to me.
