    First of all, the lessons.
Secondly, you should do something about the clause 'that I put down last time', because in your sentence it modifies the word 'lessons', whereas it should modify 'dates'. So I would restructure the whole sentence.
Last time I put down the dates of the lessons. I'm sorry I can't remember them.
Maybe somebody will suggest a better variant.

    Re: question

    Is this a correct sentence.

I have got my car washed today.
I got my car washed today.

seceondly, I feel problem where to use "It"or "This". I can't make differentiation between them.

Both of these sentences are correct.

Differentiating between 'it' and 'this' is often a hair's split process. I'd recommend to use 'this' when you actually see the thing at the moment of speaking. Otherwise refer to it as 'it'.

    Re: question

Let me add a few things on the topic.
When somebody knocks on the door, you say
    'Who is it?'.
If you saw something strange on your desk, you would say
    'What's this?'

When you introduce a new object into a situation, you say 'This is X'. When you refer back to this object, you say 'It is ...' .
This is a ball. It is small.
