    how usage

    1-I have seen Jeff.
    -How is he? Is he as good-looking as Mary says he is?

    (How=physical appearance)

    2-I went to high-school with Jeff.
    -How is he? Is he a nice guy?
    (How=character)

    3-I used to work for Jeff. He was my boss.
    -How is he?

    (How=what kind of a person is he in his capacity as boss, etc.)

    Would you say that the sentence 'How is he?' is natural in all of the above contexts?

    No, they all sound wrong. The phrase you want is "What is he like?"
    "How is he?" means "Is he well?"

    1) What do you look?
    2)What's he like?
    3) How was he?/ What was he like?

    How do you look? or
    What do you look like?
