    Dear Teachers

    [The doctor little thought that she would survive!] is weird?

    Or should I say 'little did the doctor think that she would survive!?"

    You should say "The doctor didn't think she would survive". It's useful to know the meaning of what you have written, but it would sound strange in speech.
