    Which of the following phrases is correct?

    "Once each 6 months"
    "Once in 6 months"

    I found both forms at google, so I'm in doubt. Is there any other form to say the same thing?

    He has washed the car only once in (the last) six months.

    I see the dental hygienist once every three months.
    (We would not use 'each' in this context. 'each' is used to refer to every one of two or more people or things, regarded and identified separately.
