    His request for time off work was met with a flat refusal.

    Please explain the emboldened parts to me.

    His request was met with a flat refusal. Whoever he asked just said 'no'. The request was for time off work. The words in the thread's title are not related to each other.

    "Time off work" is a period when you are away from work during your normal working hours. Common reasons for taking "time off work" are an urgent dental or doctors appointment, sickness, a funeral, etc.
