    Medical fitness certificate - self attestation

    One of my cousin she wants to submit the medical fitness certificate at office, it is a self-attestation. The message should contain as;

    I hereby declare that I am medically fit and certify that this self declaration is true and if you found that in future I am not medically fit, you may take a necessary action / termination of my services.

    Pl. help me out !!

    The message written by me very brief. Need atleast 4 to 5 lines.

    You could add 4 or 5 lines listing all the illnesses you don't have.
    But really, if they're going to accept a self-written medical certificate, all you need is a statement that you're fit and well enough to do the activity.
    If they won't accept a self-written certificate, it doesn't matter how many lines you write.
    What is the actual question on the form?

    In the form mentioned only a self attestion. Since I could compose only 2 lines, thought of taking the help of experts !!

    The more you write, the more it seems you've got to hide. If you are fit enough for the activity, and have no medical illnesses, just say that. They can always ask for more information if they want it.
