    When a mirror fell, it breaks into pieces.


    Please correct this sentence and also suggest me a good version to express the same.

    I believe the verb 'to fall' should be in the simple present tense.

    "When a mirror falls, it breaks into pieces."

    That is correct. You are stating a fact that applies to all mirrors, at least ones made of glass; you are not talking about a specific mirror falling.
