In the following sentence, All he could think to do was pray silently "for the first time in who knows how long".

What does " for the first time in who knows how long " mean ?
and this expression of " for the first time in who knows how long " is a idiom, or slang ?

Re: " in who knows how long "

All he could think to do was pray silently "for the first time in who (who in the world would ever know) knows how long".

    Also:

no one would ever be able to guess or know how long it has been since I prayed.
