    Which is correct comma placement

    Which sentence is correct and best?

    1. Unfortunately, in this case, XYZ company failed to notify us.
    2. Unfortunately, in this case XYZ company failed to notify us.
    3. Unfortunately in this case, XYZ company failed to notify us.
    4. Unfortunately, XYZ company failed to notify us.

    1 and 4 are both okay, but they don't say the same thing.

    With 1, 2, and 3, you understand that "it" has happened before and XYZ notified you those other times, but just not this time.

    With 4, you don't know if it ever happened before.
