Should it be come or comes?
I believe it should be 'come', but I often hear "It's a dream come true" and this confuses me.

Well, it could be an elliptical sentence. Then 'My dream come true' is correct. Though the phrase would sound more natural with the second person pronoun: May your dream come true. About one's own dream, I think, one would say: I want my dream to come true, or If only my dream would come true.
