    I understand the first part of # 1 differently. "baby taken out" I understood that a baby was delivered by Caesarian Section, and that the term of the pregnancy was only 6 months.

    You can see that it is a confusing sentence. No need to be embarrassed about it. I have no idea whether it is what I think or you. Clear as mud is the expression that comes to my mind about it.

    It means that the baby had an operation but in my post,I wrote that the baby was taken out.means,a pregnant woman had an operation and a baby was taken out that's why i want to ask what i should use 'by' 'through' or what else?like...
    A baby was taken out by/through an operation from a pregnant woman's tummy.

    A baby was taken out by/through an operation from a pregnant woman's tummy.
    "by Caesarian section" and if you want to say at what stage the pregnancy was you can say "by Caesarian section at 24 weeks or 6 months"

    You can say, The mother had a preterm cesarean delivery when the baby was at 24 weeks of gestational age.
