Co-Workers disagree, please correct the following-
1. Home is where patients want to be. And when your arranging long term therapy....
isn't there something wrong with the preposition AND you never begin a sentence with AND!!

'your' should be 'you are' or 'you're'. Your indicates possession not action.
I was always taught never to start a sentence with 'and' (or but) and in this case I'd agree that your sentence is poorly constructed. In spoken English it would however be acceptable to start a sentence with and...... It can be used for emphasis.
I might want to tell you that I love you. And I may want you to know that your beauty hypnotises me. And I may want you to know that I dream about you. But I'm scared to tell you these things!
The above paragraph is gramatically non-standard but in my opinion acceptable and effective use of English. I would certainly speak in such a manner.
