    Comma help!

    Hi, not a teacher!!!! I am involved with medical transcription. I have a sentence that I have been tossing around as to the right way to transcribe. It is: Nocturia four to five times without history of recurrent urinary tract infections. My question is should I type it like this: Nocturia, four to five times, without history of recurrent urinary tract infections.
    (commas or no commas is the question basically).
    Please help!!!

    The fuller, 'understood' sentence would be:
    The patient has experienced nocturia four or five times, without any associated history of...

    Hence, punctuate it:
    Nocturia four or five times, without...
