Smile Would you sign in this receipt?

Hello~ Can you help me say this naturally please?

    At the computer service center.
A: ⓐI think my computer is broken. I canít boot it.
ⓑI think my computer has broken. I canít boot it.

B: Ok, you better leave it for couple of days, we can check it.
ⓐWould you sign in this receipt?
ⓑWould you make a signature on this receipt?
And, you should bring it with you when you get your computer back.

Re: Would you sign in this receipt?

I am not a teacher, but I am a native speaker.

A: Both of them work, although I think you would probably hear "is broken" more often.

B: Neither. It is better as "Would you sign this receipt?" or "Would you please sign this receipt?" (more polite).
