    Hello~ Can you help me say this naturally please?

    A: How often do you drink?

    B: ⓐAbout once every 3 days.
    ⓑAround once in 3 days.
    ⓒRoughly once a 3 days.

    a or b; c would be possible only in a jocular context where the pattern 'once a <period>' had been established:

    'How often do you go - once a year? once a month? once a week?'
    'Once a three days, actually'. [This is wrong, but could be used as a joke (and some linguists would object to my use of the word 'wrong').]

    For (d), as the frequency is approximate ('roughly/around/about') you could say 'twice a week'.

    Speaking of jokes, that reminds me of a Fawlty Towers routine:
    Psychiatrist Guest, (unbeknown to Basil, speaking of getting away for holidays): How often do you manage it?
    Basil (defensively): Pardon?
    Psychiatrist: You and your wife. How often do you manage it?
    Basil (op cit): Oh ... about average ...
    Psychiatrist: Once or twice a year, I imagine.
    (Basil looks perplexed and vaguely offended as usual).

    Can I ask one more question?

    When it's not a jock, when it's serious how should say?

    For example,

    I ask doctor: How often should I take this pill?
    Doctor: Just once every 4days.
    And you should take shot once every 3months for 2 years.

    Is it correct?

    I'd say: "Just once every four days. And you should take a shot every three months for two years from now."
    Is this correct?
