    How different are these sentences:

    Have you ever been to Africa?

    Have you been to Africa?

    I'd say that semantically, there's no difference in meaning; gramatically, the only difference is that you used the adverb ever in the first sentence.

    However, there is one context in which the two questions are quite different:
    [your friend has just returned from Africa; the friend told you nothing about the trip, but you were somehow able to find out; now you're trying to investigate and confirm the information:]
    Have you been to Africa? rather than Have you ever been to Africa?
