Can "a" be used before Mr. Mrs. and Miss?

A Mr. XXX came to meet you.

Yes, it can. It means a person by the name of Mr ABC came to meet you.

Note, I used ABC because alas XXX has acquired a special connotation. http://www.yourdictionary.com/xxx
