One of my great friends, who is an English teacher in our institute, wants to teach a course about airplanes and flying and stuff. Because he is an old man and not in the mood for web-surfing, he asked us to find the forms every person in an airplane has to fill for identification.
I'm searching for the names and pictures of any form the passengers have to fill when they enter an airplane.

In most countries, you don't have to fill in a form to board an aircraft. I have had to complete an immigration departure form and a Customs form in only two of the countries I have visited.
You usually have to fill in a form to present to immigration/Customs at your arrival airport and sometimes they hand out those forms on the plane to complete before you disembark.

You need to do some work here. Just search for "immigration forms" and "Customs forms" in Google Images and you will get plenty of pictures of them from all over the world. Bear in mind though that there will be no way to tell if those forms are required in order to board an aircraft.
