What is the exact meaning of "all together now" expression?

Maybe something like "Here we are to do that all together, at once? Much in the sense of "Union is the strength"?

Yet another possible meaning is an expression of confidence that other people will automatically think the same, or an expectation that they should. "And what do we say to pushy salesmen at the door? [Aside, to the audience:] Altogether now - [leading a chorus of expected words] 'Not today thank you'".
