I am wondering if my sentence is grammatically correct.

Andrew not only had to work in a restaurant, but also had to take care of his old mother, who he did not want to put in a care home.

Instead of "care home", I would use, e.g., "nursing home" or "home for the elderly".

Andrew not only had to work in a restaurant, but he also had to take care of his old mother [STRIKE],[/STRIKE] who he did not want to put in a care home.

Care home is fine and no need for a comma after mother.

Can I use "have" instead of "had"?

You can't use "have" but you could use "has" in place of "had" both times. You would need to change "did" to "does" at the end.
