what's the difference between "competition" and "contest"?
Much of the time there really is no difference. A competition is a contest, and a contest is a competition. However, contest does not share competition's more general meaning of struggle for supremacy as in the sentence, "Competition between businesses is good for the consumer."

A form would be a document to be filled in with information, as when applying for something. a chart is something like a graph- pictorial representation of information. ;-)
