assessment/performance charts

I have EFL conversation clases and writing classes. I'd like to make a performance chart of sorts, i. e., showing whether students' have performed tasks adequately and what ,if any, areas students' need to review,etc.

Any ideas of where I might find an already made chart or sheet, or
to make one?
