1. This means "at any particular time". For example:
"At any given time, millions of people are using the Internet." This means that at any specific time we wish to choose, millions of people are using the Internet.

2. It can be used like this:

"We had a bit of a problem yesterday." This means:
"We had a problem, but it was not very serious."

3. It means he played very badly. It does not mean that he did not "play fair", it simply means that his actual playing was very bad.
