1) He always mistakes my name for your names.
2) He always make mistakes when it comes to name calling/calling name.
3) He called my name by your name instead/ he called me by your name.
4) Don't be too noisy otherwise you will wake the neighbours up.
5) I was woken up by the sound of the phone ringing.

2) He always makes mistakes when it comes to name calling.(If you mean a derisive remark)
3) He called me by your name instead. OR He called me by your name.
4) Don't be too noisy otherwise you will wake the neighbours up. OK
5) I was woken up by the sound of the phone ringing. OK
