"whom do I make it out to?" or sth like this. I heard it before, but really not sure the accuracy of the sentence. Sorry, maybe I shouldn't have even tried.

Yes, "whom do I make this out to" is fine.

Also, "Who is the payee?" (YOU are the payer)
