That sounds formal. Try "I'm so sorry to hear of your loss" and/or "My deepest sympathies."

I have certainly commented "My condolences" after hearing of the death of a friend's relative/friend.

You can speak the words "my condolences", as emsr has pointed out, but you can't say condolences.

You can give condolences as in emsr's example. Another slightly more formal and longer version is "You have my condolences."

Admittedly, I wouldn't say "My condolences" if I were talking to the bereaved person face-to-face. If someone said to me "My mum died yesterday", I would say "I'm so sorry."
I would use "My condolences" in an email, a text or in a Facebook post.

I would write Please accept my condolences. If speaking, I would use Teechar's suggestions.
