You have to restructure the sentence.
Could you please tell me the reason [that] she feels so offended?
Could you please tell me why she feels so offended?
Could you please tell me what made her feel so offended?
Could you please tell me what it was that made her feel so offended?

You can't phrase it the way you have done because 'a reason' did not make her offended.
