2- If it is ok by him?
3- It is ok with me.
4- It is ok by me.
5- There is not problem in starting the twelveth.

• 1- Yes, but it's a little strange to make it a question. You could do it in speech with rising intonation. In writing, you'd need some context to make it a question.
  2- By can also be used.
  3&4- both are fine. At least in BE, the prepostioins can be used. Maybe it's different in AE, we'll havce to wait for RonBee to drop in to confirm this.
  5 Use 'no' to make a noun negative- There is no problem in starting (on) the twelfth. Also, note the spelling of 12th.

I'd probably use 'by' where the person had the power to give or deny permission. ;-)

If it's OK by him = If it's OK by his authority/word.

As far as I know, it is the same in AE. Tdol's (and Cas's) advice is okay by me.
