Which one is correct, fill in or fill out? My answer is (a)

a) Can you fill in your date of birth, your student ID and the year you graduated from the university?

b) Can you fill out your date of birth, your student ID and the year you graduated from the university?

As an NES but not a teacher, I would probably fill in individual boxes when filling out a form.
Others may use either, depending on their own personal preference.

I would say that you fill in the information while you fill out the form.

I would fill in a form by filling in the individual boxes.
