As far as I know, this expression is only used in marriage vows and means "whether things are good or bad".
Is it used anywhere else and does it have any other meaning?

It can be used in other situations of a similar nature, with the same meaning, like partnerships, etc.;-)

there's a comic called for better or worse.

its posted in the Vancouver Sun and THe Province (candian newspaper)
