In formal English letter writing, where the source address is at top centre or top right of the page, it is not normal to include your name,

For example:

42, Greyhound Road
Perry Barr
Birmingham
B42 6HJ

Mr. E. Scrooge
The Manager
Barclay’s Bank Ltd
113 Mammon Street
Andover
HU4 9ET

It may be that practices in the USA and in the UK are different in that respect. I would always put my name there (above the address).
