From most to least natural: 1, 4, 3, 2.

The simplest form is usually the most natural. Notice that 1 is the shortest.

Are these sentences correct and natural?

1)Feedback was anonymous to encourage openness and honesty in participants. This one is correct and natural.

2)Given feedback was anonymous to encourage openness and honesty in participants. The is not correct and not natural

3)The feedback which was given was anonymous to encourage openness and honesty in participants. With the change shown, it's correct, but it's not natural. It somehow suggests there was other feedback that was not anonymous.

4)Feedback given was anonymous to encourage openness and honesty in participants. There is no point in including "given."

See above.
