Have you ate bnana?
is a wrong sentence

Have you eaten banana?
is right sentence

When we use Have(Aux verb) it is followed by past participle of the main verb.

Have( aux) you eaten(main verb)

Lets wait for any teacher.

(1) May I just add that you probably need an article.

(a) Have you ever eaten A banana? (You want to know whether that

friend has ever eaten a banana at least one time in his/her life).

(b) Have you eaten THE banana? (There was a banana in the kitchen this

morning. Now it's not there. You want to know whether your friend ate

it. )
