it's correct to say:

My annual leave is as follow:

Please follow the rules of written English:

- Start every new sentence with a capital letter.
- End every sentence with a single punctuation mark (full stop, question mark, exclamation mark).

I have made some changes to your post in red.

If your annual leave has been agreed, then "My annual leave is as follows: 11/6/13 to 10/7/13". However, if you are requesting those dates (your manager has called it your leave plan) then you should say something like "I would like to book annual leave from 11/6/13 to 10/7/13".
