How to write an attest to the airport customs

Hi!

Can you kindly advise me how to write an attest for the airport customs to prove that the medication I'm taking're needed for my illnesses i.e.retinal thrombosis at left eye and a carotid operation was done at the right side of my neck.

Unfortunately,my German general practitioner's not able to make this translation.
I have to make this translation and my gp'll official it with his signature.

Therefore,I need your help in "how to begin and end" such formal mails.

Have your doctor write the attest in German. Do your translation and post it, and we will then be able to advise you.

Since this is a general notification, it can start

"To Whom it may Concern".

The end only needs the doctor's name and signature - so other ending.
