I am thinking at five in the morning (the day on which I have to go to the hospital) "if I had eaten the food by 10:30 PM 'it would already be six hours fasting by now' or 'I would have already fasted for six hours by now'".

Please check my sentences.

"If I had eaten at 11pm, I'd have been fasting for six hours by now".

Your maths was out. If you ate at 10.30pm, by 5am, you'd have been fasting for six and a half hours.
