"the book" is somewhat redundant. The word "read" makes it fairly clear it's a book.

Have you read The Old Man and the Sea by Ernest Hemingway? (No commas are required.)

Even if you include "the book" you don't need any commas or the word "titled".

I have no problem with your use of the word "book" in these sentences. It is not redundant since not everyone knows this is a book. It could be a poem.

The only word I might use to replace "book" would be "novel". This would alert the reader that the writing was not a short story or "novella" in case they had no knowledge of the writer's work.

As far as punctuation and font for the title goes, this is a matter of style. If you are writing this sentence for a school project you should ask your teacher which style you should use. Italics, quotation marks or even underlining can be used. Underlining a title was done more in the day of the typewriter, not the computer. Typewriters had no means of changing the font of the writing.

So these forms are acceptable as far as I know. (There should be no commas used.)

The style you would use would depend on who you were writing the piece for.
