Which of the following tenses would be correct:

Please keep me posted if you decide/ decided/have decided to do so.

Which of the following tenses would be is correct?

Please keep me posted if you decide/decided/have decided to do so.

Please note my changes above. The most natural is "decide", which is used here to refer to the future. It's possible to use "have decided" if you think the person has already decided but hasn't yet told you their decision. I wouldn't use the simple past.

You're talking about someone making a decision in the future, so present simple is correct. I don't think that keep me posted is quite right, however, because it means something like 'give me a continuous update'. Since you're simply asking someone to tell you just one piece of information, I'd suggest:

Please let me know if you decide to do so.
