Yes, I believe they are grammatically correct. However, I believe it is a little "off". Perhaps:

  • [list:18693da0ad]Please look through the form to make sure that all the entries are complete and correct. If you are satisfied that everything is in order, sign and return the form to me.

(Check is not usually used with through but instead is used by itself.)
