The phrase (it's more than one word) means to start completely from the beginning.

If you were working on something and stopped, and then came back and realized it was wrong, you may have to start from scratch by doing the very first thing over again. On the other hand, you could "pick up where you left off" (start just at the same point you had previously stopped) and try to correct it along the way.
