Break it into two separate questions and see which seems to make sense:

Have you received the sample?
Do you .................. to pay the supplier?

Would you use need or needs?

Why did you choose "needs"?

The verb "to need" follows the pattern of all other regular verbs. "You" is the second person singular and that has no "s" on the end.

I need
You need
He/she needs
We need
You (pl) need
They need

So - why did you choose "needs"?
