I think, there is a subtle difference between both meanings, yours and mine one. In my opinion, it is quite possible to put in overtime work without extra pay.

off-the-clock = overtime without extra compensation.

"She often has to work off-the-clock."

(Some work places have time clocks where people clock in and clock out)

clock in and clock out

Begin work, as in She clocked in late again. Also, clock out, end work, as in Please wait for me; I forgot to clock out. The allusion here is to punching a time clock, a device that punches the time on a card to record when an employee arrives and departs.

In my opinion the meaning of "off-the-clock" in the present case is " hold more than one appointment". In our country we describe this as "off-hours-job" ("extra, additional work"). It is of no consequence whether there is a work place with time clock or it's missing such one. The time clock is mere a form.
